A formal diagnosis of autism by a pediatrician or other qualified health professional is required prior to beginning services.

Once these forms have been filled and signed they can be emailed to [email protected] , mailed directly to our Tifton location at 1644 HWY 82 Tifton, GA 31794, or submitted online via the “Submit” button below.
